MEMBERSHIP APPLICATION INFORMATION

Ice Hockey Australia (IHA) is the trading name of the Australian Ice Hockey Federation Inc (AIHF). All reference in this application to IHA is to be taken as reference to AIHF.

The information requested in this application by the Club (Falcons Ice Hockey Club) is on behalf of the State Association and IHA. IHA, State Associations and Clubs require the information requested below to provide you with membership services.

Your personal information will only be used in accordance with the objects and purposes of IHA, the State Association and the Club.

You will be able to access your personal information through IHA, the State Association and the Club upon reasonable notice.

If the requested information is not provided you will not be able to receive membership services.

I hereby apply for affiliation with IHA and permission to participate in IHA sanctioned events and competitions.  In so applying and in consideration of my application being accepted I acknowledge and agree that: for the purposes of this declaration “IHA” means and includes Australian Ice Hockey Federation Inc, its member Associations (IHSA) or Clubs (Falcons Ice Hockey Club) and their respective directors, officers, members, servants or agents.

FEES

You agree to pay all fees set down by IHA, the State Association and the Club in a timely manner and that you are responsible for those fees at all times.

FIHC 2018 Winter Season Fees - $TBA

Upon registering you acknowledge and agree to pay the season fees in full and before playing your first game.

You also agree that non-payment of your fees may result in the termination of your membership and your exclusion from any games sanctioned by IHA until such fees are paid in full.

RULES
I agree to be bound by the IHA Constitution, any regulations made under that Constitution and my State and Club Constitutions; and I acknowledge and agree that:
• The IHA Constitution is made in the pursuit of a common object, namely the mutual and collective benefit of IHA, the members of IHA and Ice Hockey; and
• The IHA Constitution is necessary and reasonable for promoting the objects of IHA and particularly the advancement and protection of Ice Hockey; and
• While I am a registered member of IHA, my membership may be cancelled if I participate in any Ice Hockey Competition that is not sanctioned by IHA.

WARNING
Ice Hockey can be inherently dangerous and that serious accidents can and often do happen which may result in me being injured.  I declare that I have voluntarily read and understood this Warning and accept and assume the inherent risks in Ice Hockey.

EXCLUSION OF LIABILITY
Except to the extent that the Trade Practices Act 1974 (Cth) or other legislation applies, and cannot by contract be excluded, I agree that it is a term of my affiliation with IHA, that IHA is absolved from all liability however arising from injury or damage however caused (whether fatal or otherwise) arising out of my affiliation with IHA, and / or participation in any IHA authorised or recognised activity or in any way due to any negligent act, breach of duty, default and / or omission on the part of IHA, its Associations or affiliated Clubs.

RELEASE AND INDEMNITY
In consideration of IHA accepting my application for affiliation I:
1. Release and forever discharge IHA from all actions, suits, proceedings, claims, demands, penalties, costs and expenses however arising that I may have or may have had but for this release arising from or in connection with my affiliation with IHA and / or participation in any IHA authorised or recognised activity; and
2. Indemnify IHA and its Associations or Clubs, to the extent permitted under the Trade Practices Act 1974 (Cth) or otherwise by law in respect of any actions, suits, proceedings, claims, demands, losses, damages, costs, expenses, penalties and fines arising as a result of in connection with my affiliation with IHA and its Associations or Clubs, and / or participation in any IHA authorised or recognised activity whether caused or contributed to, directly or indirectly, by any act or omission (including negligence) on the part of IHA.

FITNESS TO PARTICIPATE
I declare that I am medically and physically fit and able to participate in any AIHF authorised or recognised activity including AIHF Ice Hockey Competitions and associated trainings. I have read, understood, acknowledge and agree to the above declaration including the Warning, Exclusion of Liability, Release and Indemnity.
